Personalised care planning

We develop each Support Plan collaboratively with the person, their family, advocates, local authorities, ICBs and healthcare professionals, always guided by the individual's own agreed outcomes.

Transitions

A smooth move into a new home.

Moving into a new home is a significant life event. We develop individualised transition plans tailored to each person, from extended introductions and joint working with current providers, to urgent arrangements within 24 hours.

Our priority is ensuring each person feels safe, informed and genuinely supported throughout the process.

We use a range of tools so each person can understand their new environment, routines and support in a way that is meaningful to them.

Monthly key-worker reviews

Every month, individuals meet with their key worker to review support, progress and share feedback.
